What should I bring to my appointment?
Bring your completed patient form, current glasses or contact lenses, a list of medications, and your insurance information. Call us if you have questions about preparing for your specific visit.
How do I book an appointment?
Use our online appointment scheduler or call (435) 283-5555. Our regular office hours are Monday through Friday, 8:30 am to 5:00 pm.
Do you accept my insurance?
Our office works with major insurance plans, but benefits and participation vary. Call us before your visit with your plan information so we can help confirm the details.
Can I complete my forms before I arrive?
Yes. Download the new patient form, print and complete it, and bring it to your appointment. The form includes personal information, so please bring it directly to the office.
