Once downloaded, please print and fill out the necessary information on the forms. Remember to bring these completed documents with you to your appointment, ensuring a smoother and more efficient experience.

Insurance and Payments

They offer various financing options to make eye care affordable, emphasizing no patient should forego eye health due to cost concerns.

  • Other Payment Options: Discussing financing options before examinations is welcomed to ensure cost isn’t a barrier to eye care.
  • CareCredit: Recognized nationwide, CareCredit serves as a patient payment program, facilitating financing for eye care services.
